Can’t complete the form W-8BEN right, they keep returning it tome.

Would you please help me and tell me what I complete wrong. And what you put there if you are in Canada. Line 10 I wrote: Article 12 paragraph 3. Is this correct? 
How did you explain what you do and additional conditions?
