Didn't receive payment yet because of the w8-ben form

Hi dear Zazzlers and dear Zazzle team,

I have reached payment treshold a while ago, but I have orange notice box that says “W8-BEN form has not yet been collected”, so my earnings can’t get through. I sent w8ben form on 22nd of july, and again at the beginning of september with no response. Now I am worried it might be lost somewhere.

If someone from Zazzle team sees this, I would be very grateful if you could look into this matter.
