Moderators please help me to find out about my W8-BEN form

Moderators would you please help me to find out about my W8-BEN form, I assumed it was alright and accepted as only a date was missing to fill out for me I was told. I haven’t heard back after. Would you please ask on my behalf if it was accepted for sure. It’s been a month since I haven’t heard back and I asked myself but no answer back unfortunately. Thank you.

can you post what you used W8-BEN form if your from Canada thank you
