I received a paper copy in the post for end of the tax year. My date of birth is wrong and is not what I sent to Zazzle.
Any reason for this?
I am in the UK, should this be a concern?

To be sure - is the month written out or are month and day both numbers? If numbers, are the month and day numbers switched?

Everything is completely off.
I’ve also had this happen on a past tax slip. I wouldn’t worry about it.
Mine too. Also my tax number is wrong and I have apparently earned nothing.
I spent months changing tiny details on the w8 form to make it fit the exact criteria zazzle demand and this is what you get in return.

Add me to the list. In the UK - just received 1042s in the post and DOB incorrect (and like @Zob1 wish I was that young)

Hi! I just received the 1042-S form and I have no idea what to do with it. My birth date is completely wrong too, and the tax number isn’t even filled in. Do I need to do anything about this? I’m from Brazil, by the way.
Same issue here. Date of birth, foreign tax ID number, and gross income are all off.
Mee too. I received it with wrong data and immediately reported to Zazzle. Waiting for better news. From informations I have from Zazzle, they are working on it. So, it seems there are too much people outside of Zazzle more remaining in the illusions and don’t apply to understand the real situations. Please do not think more I’m a stupid (Such as I have to check my browser…) where I write/report something… Thank you
My DOB is wrong too. I am in Belgium.
Something more about the concern. I’m completely ok with my country tax purpouses. I usually received a copy of 1042-S form from Zazzle too, and it was correct. I’ve reported immediately to some Zazzle agent, but I’m still waiting answers from them.
1042-S I received last year had the wrong DOB. I contacted Zazzle, they told me they are working on it, and I haven’t heard back from them again. I still haven’t received 1042-S this year, but reading all your comments, I am pretty sure DOB will be wrong again.
I asked and reported to support much times about my earnings form. Finally, they updated it now about me. But they didn’t give clarifications about that.
