Removing items from Collections doesn't work

I’ve followed the instructions to uncheck the box in the same way I checked it in the first place, but things remain in collections (where I mistakenly put them). Is this a system-wide problem? Something that gets updated e.v.e.n.t.u.a.l.l.y? Or am I doing something wrong?
