When asked what to write for Part 2, Line 10. Add "ARTICLE 12, PARAGRAPH 3"
When asked to add "Additional Conditions" on the last section in line 10. Add "Creation of artistic works (designs) sold on Zazzle online web stores on a range of goods"
See the screenshot from my form. I've submitted it today after about 10 attempts to fill this in. Bearing in mind I've been selling on Zazzle for 5 years and my previous form, without these details, was accepted for the past 5 years!

For some feedback, Zazzle really needs to provide more simple ways to fill out this form. I have had to resort to reading endless entries on the public forums on Zazzle to get help, rather than using the links I keep getting from your team."
