For the benefit of the many hundred who ask queries re W-8BEN forms here and on social media groups, I thought a very simple resource would help.
Please follow the rules of this thread to ensure it fulfills it's purpose which is to provide an EASY FAST route for Zazzlers who have had their forms declined to find the CORRECT way to complete a w-8Ben form for their specific country.

1. FIRST CHECK TO SEE IF A FORM FOR YOUR COUNTRY HAS ALREADY BEEN ADDED HERE.
If the form was approved and accepted there is no need for duplications.
2. REMOVE The personally identifiable data or replace with placeholder information.
3. DO NOT leave comments NOR ask questions in this thread (better to start another so as not to distract from this being a useful resource).
4. ADD the country FIRST in largest size RED for easy location.

The date formats are AMERICAN so the MONTH should come first for D.O.B and Date of completion ie:
04/31/2024 MONTH/DAY/YEAR
All name and address details need to mirror EXACTLY what is on your Zazzle account. If your account details require updating contact Zazzle support to have them updated first!
