images missing again

images are starting to not show up in the media folder again. Also, the 'My Albums' folder in image browser are still jacked up. Images are there but not there. also sometimes the image name is replaced with a long string of characters instead of its name when placed onto merchandise.

Addendum: ~10,000 products moved to 'hidden' status in the last few days by automated systems.

Addendum 2: backend- no products showing past page 49 with [show 204/page] setting.

49 pages of 204 per page are only 9996 products that can be accessed on the backend for any search criteria. if our store capacity is still 100,000 that would be only 10% of the store that can be accessed. please advise
