Can't receive payment because of the w8-ben form

Drear Zazzlers and the Zazzle team, I hit the payment threshold a while back, but I haven't received my payment yet due to some issues with the W-8BEN form. I submitted it twice - the first time it got rejected because "the address in my account didn't match the one on the form". However, I made sure to input the correct address on the form. The second time, I got a message saying, "Unfortunately, the W-8BEN form you attached to the email did not include any of your information." Even though I filled out the form with all the necessary details, so I'm not sure why my earnings aren't coming through. Now, I'm worried that it might be lost somewhere in the process. If someone from the Zazzle team reads this, I would really appreciate it if you could take a look into this matter. Thank you!

The last time I did it, the form was read only so couldn't be saved with the info added. So I printed it, filled it in, scanned it, attached it as a picture. That was accepted.

Thank you for your advice. I will try it. Even though the first time I printed it and wrote the details by hand, the second time I wrote directly on PDF. I will try again, and I hope it works
