W8-BEN Question

I sent the final version of W8-BEN forms on April 21, 2023 and since then I haven't heard anything.

Hello, when I first sent my W8ben form in april, I received a reply from zazzle after waiting for two weeks! It wasn't accepted because my payments settings on zazzle weren't complete... I completed/updated them last week, but my paypal E-mail just kept disappearing after I clicked Save! I really don't understand why! I sent an email last week for help, to help me save my new settings, and I said that soon I'll send the new w8...and still no answer! I suppose we will have to write a lot of letters....until someone decides to answer.....................

I'd be really thankful, if someone from the staff would help with this w8ben form And the weird Payment settings in account which aren't saved (only my address was saved, but not the paypal email) !!!

I have the same probplem from March! After the two weeks of waiting the reply , the answer was, that my name and adress doesn't mach. (Ok, I thought that the problem is with me...)The other problem was the percent of the royalties, but when I checked the document (which I was using!) everything was fine! Ok, I refilled everything again, this time with copy and paste for the name and the adress and everything! And the reply, again, was that they are not the same! 
One of my mistakes was, that I didn't wrote down in part 2 "Rolyalties.." (now I saw the exact sentence so I will correct myself). But strange, the other things was how they are! 
The second very strange problem with my account is the same as above me...I fill my payment details and after that I recieve a mail...and after this, everything disappears ! I thnought the problem is technical and I refilled everything again, but the result is the same. 
I have some questions:
Is this because of that I haven't sold anything yet? Is it possible that, after my first sale everything to be activated? 
(My stores are made from several days priviate because I have to do more things on them and also I am not sure what to do before I activate my account.)
Is there somthing specific (as letters, way of writing ) in the filling of the form about the adress and the name?
Could it be possible to see w8-ben form as an exaple, to be clear what is the right?

I have same issue, with w8ben form. i sent file 2 weeks ago, received reply it doesn’t contain any info except signature. On sent mails i checked and everything is ok. 😞 I have resended again and don’t know when it will be approved.
