Can anyone in the UK please help me with filling out Part 1 of the W-8BEN ?

6a. 'foreign tax identifying number' - is that my 10 digit UTR number?

7. 'Reference number(s)' - what goes here?

Yes it should be your UTR number under 6a.

Under point seven I've added my Zazzle member ID which is the same as your associate ID, which can be found here https://www.zazzle.com/my/associate/associate  But apparently you can leave this blank. I have seen forms without it being accepted.
